If you would like to register with Oasis Primary Health Care Clinic, you will need to sign the roster form. When you sign this, you will be rostered under Dr. Preston Zuliani although you will being seeing the nurse practitioner, Laura-Dawn Moule.
If you have insurance or would like to pay for your health care services, you do not need to sign the roster form. Just call 905-931-5151 to book your appointments. You will not have any restrictions on the use of walk-in and methadone clinics.
